RN Patient Care Coordinator
Location: Queens, NY
Schedule: Monday–Friday
Position: Full-Time, Onsite
Salary:100K annually + Comprehensive Benefits

Are you an RN looking to move beyond traditional bedside nursing while continuing to make a meaningful impact on patient outcomes?
We are seeking a Patient Care Coordinator – RN to provide proactive, patient-centered care to a high-risk patient population. This role works closely with providers to coordinate care, develop individualized care plans, educate patients, and help address barriers to treatment.
Job Responsibilities:
- Complete pre-visit planning and review daily patient appointments.
- Conduct proactive outreach to high-risk patients.
- Coordinate care and develop individualized care plans with providers.
- Assist patients with treatment goals and identify barriers to care.
- Educate patients on their care plans and available resources.
- Accurately document patient interactions and care plans in the EMR.
- Participate in Quality Improvement initiatives.
- Collaborate with Benefits Counselors, Outreach staff, and other healthcare team members.
- Perform other duties and special projects as needed.
Qualifications:
- Valid New York State RN License required.
- Prior clinical experience preferred.
- Care coordination or administrative experience preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to work effectively with providers and multidisciplinary teams.
- EMR experience preferred.
